Pre-care of Patient with Dialysis
· Explain procedure to family and family
· Encourage family and patient to verbalize fears and concerns and to ask questions
· Assess Weight, blood pressure, heart rate and rhythm, respiratory rate and quality, temperature and dialysis vascular access.
· Assess patient’s general condition including edema, skin, mental status and ability to ambulate
· Evaluate complains like dizziness, headache, fever, chills etc before dialysis
· Review laboratory test results prior to initiation of treatment.
Intra-care of Patient with Dialysis
· Monitor vital signs every 15min initially and subsequently at hourly intervals
· Notify physician if any sign of bleeding, severe abdominal pain and respiratory distress
· Monitor serum electrolytes, blood chemistries, hematology studies
· Use strict aseptic technique to avoid peritonitis
· Assess patient frequently for pain/discomfort
· Assess dialysis drainage: cloudiness suggests peritonitis
Post-care of Patient with Dialysis
· Perform PD catheter site care
· Assess for signs/symptoms of infection: redness, swelling, tenderness
· Remove dried blood and drainage
· Cleanse exit site gently and cover site with sterile gauze and secure dressing
