Myocardial Infarction
Myocardial infarction occurs when ischameic intracellular changes become irreversible and necrosis results. Angina as a result of ischaemia causes reversible cellular injury and infarction in the result of sustained ischaemia causing irreversible cellular death.
Clinical manifestations
· Chest pain
· Nausea and vomiting
· Sympathetic nervous system stimulation is increased
· Fever
· Arrhythmias
· Congestive heart failure
· Cardiogenic shock
· Papillary muscles dysfunction
· Ventricular aneurysm
· Pericarditis
Nursing Management
· Ensure patient airway
· Administer oxygen by nasal cannula
· Insert 1 IV catheters
· Obtain 12-lead ECG
· Determine location of pain
· Medicate for pain as ordered
· Obtain cardiac enzyme level
· Administer asprin
· Monitor vital signs, level of consciousness, cardiac rhythm and O2 saturation
· Reassure patient
· Prepare for CPR, defibrillation
